You can fill out the following form online before printing. When finished, please print, sign and submit your form(s) to the preschool office.

Page 1 Admission Information
All five sections should be completely filled out with the exception of “date of withdrawal” in section 1. If your response is “none” or “does not apply”, please write None or N/A in the blank.
In section 2, Note:
- We do not “transport” children in vehicles. We only supervise them on field trips, not to and from home or school.
- We do not use pools for water activities.
- In #4 the operational policies will be provided at the beginning of school for new students. Signing here now will ensure that you will receive a parent handbook at Meet the Teacher day.

- Section 1 must be completed by a professional with them signing below the box or a copy of the record can be used if the health professional’s signature is included.
- The parent’s signature in section 1 is for Chicken Pox verification only.
- Please read section 2 carefully and check the appropriate boxes and get physician’s signature where needed.
- If vision and/or hearing have been screened, please have this documented and turned in for four year olds.
